Why we collect data

The personal information we collect and use is provided to us directly by you in order for me to provide you with an individualised nutrition and dietetic service. 

You may request details of your personal information which we hold about you under the General Data Protection Regulation (GDPR).  

Under the GDPR, the lawful bases we rely on for processing this information are:

·      Your consent.  You are able to remove your consent at any time.  You can do this by contacting me on abdietetics@gmail.com.

·      We have a legal obligation. 

·      We have a vital interest

·      We need it to perform a public task. 

We will only share your information with relevant health care professionals (only where consent is given by the client).  

How your personal information is stored

Your information is securely stored on Healthie. 

We may collect the following information: name, DOB, age, gender, contact details (email, phone number), marital status, living arrangements, occupation, home address, GP name and address, all relevant health and clinical information.  

We require this information for professional clinical record keeping of client information in accordance with the British Dietetic Association Guidance for Records and Record Keeping and will be stored for 8 years in line with this guidance.

Controlling your personal information: we do not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.

Under the data protection law, you have right including:

·      Your right to erasure

·      Your right to restriction of processing

·      Your right to object to processing
